Varodem explanation forms on compression aid order forms!
Varodem now offers its own simplified explanatory forms for applications to certain health insurers. The forms have been reduced to a single page and contain only the essential information. Each product group (VaroCare, Curetex, and TinkFit) has its own specific form, so you only need to answer the relevant questions. Be sure to fill out the explanatory form completely!
On this page, you will find the most recent Varodem forms and order forms. Orders can only be placed by registered Varodem customers (suppliers). The forms can be filled out digitally. Always open the forms in a PDF reader. Order forms cannot be filled out in a browser. If you have a separate reference, you can attach it as a PDF to your order.
